Registration for Preschool Screening

Butler School District 53 will conduct a free developmental screening of children ages 3 to 5 years old. The screening will take place on Thursday, October 12th at the Oak Brook Park District, 1450 Forest Gate Road, Oak Brook, IL. Children should live in the district and not be enrolled in Kindergarten. Registration will be open from September 21st-October 5th. The screening process identifies how the child is developing. Assessed areas include speech and language, large and small muscle skills, social/emotional development, hearing, and vision. The screening takes about 60 minutes. Screening results will be shared with parents.

In order to sign up for preschool screening please follow the below steps:

1. Complete the basic student information form about your child. 
2. Sign up for an appointment for the screening. Scheduling website.
3. Print out and complete the Parent Questionnaire. Bring this form with you to your appointment
